Transition from paediatric to adult services

Transition is the term used to describe moving from the children’s cardiology service to the adult service. There is no need to be nervous about this process, the specialist transition nurses will provide you with full support and are on hand to answer any questions you may have. Transition is a period during which you become more involved in dealing with your heart problem and more responsible for your own health. It is inclusive of your entire journey into adulthood and associated decisions, such as starting relationships, going to university, learning to drive or going travelling.

Your age at the transitional process will vary depending upon your medical and developmental status. Typically, the transition process will start at 13 years of age, but this is a very individualised process and can be at any time from 12 -16 years old.
